What is Colorectal polyp?


Colorectal polyp also called colonic polyps, it is an abnormal growth of the cells appear on the lining of the colon or large intestine, mainly this growth is not harmful but sometimes it may prove fatal.

Intestines is the part of the digestive system, digestive system is divided into three parts

  • small intestine – it is about 20 feet long, it absorb the nutrition from the food .
  • large intestine(colon ) – large intestine about 5 feet long ,it is hollow long tubes and situated at the bottom of the digestive track and the function of the large intestine to make and store stool.

Types of polyp 


Mainly it is two types of polyp 

  • Neoplastic polyp – in neoplastic growth two types of colon are found Serrated and adenomatous polyps- these are the most common type of polyp ,and if left untreated it leads to colon cancer, the majority of the polyps arise in the right side more prone for developing into cancer  
  • Non neoplastic polyp –this group include hyperplastic ,hamartous and inflammatory polyps -these polyps are harmless and not develop into cancer .this polyp is related with ulcerative colitis and crohn’s diseaseRisk factors
  • Age –polyp mainly occur in older age mainly above 40 
  • Medical conditions – some inflammatory diseases of the intestine such as ulcerative colitis and crohn’s disease 
  • Weight – people who are overweight are more prone for these conditions 
  • Hereditary – sometimes polyps occur due to inherit genetic mutation , some hereditary disease can cause polyps such as Lynch syndrome ,familial adenomatous polyposis , gardner’s syndrome ,serrated polyposis syndrome

Some screening examinations are 

  • Colonoscopy – in this procedure a long thin tube with a camera and light fitted ,inserted in ractum to find out the polyp ,if the polyp found some surgeons removed the polyp at the same time or taken some sample of tissue for further examination, to rule the cancerous condition
  • Sigmoidoscopy – the method is of sigmoidoscopy is same but , this test is done to see the rectum and lower colon. But polyp is not removed in this process 
  • CT colonography(virtual colonoscopy )-this test used CT scans to take images of rectum ,is also done for seeing the rectum in 2D or 3D 
  • stool examination under a microscope to rule out the invisible blood in the stool
  • Barium enema – in this process doctor insert liquid barium in rectum  and by using x ray take images of your colon 

Treatment


Conventional  treatment is removal of  polyp , majority of the polyp is removed during the process of colonoscopy

It is an autoimmune disease in which the body’s own immune system starts attacking the connective tissues. The exact cause of polymyalgia rheumatica isn’t known.

This suggests that there may be an environmental trigger, such as a viral infection, that causes the condition. The rapid onset of symptoms also suggests that polymyalgia rheumatica may be caused by an infection.

What is varicocele ?


Varicocele is the enlargement of the veins ,which is present in the scrotum ,the scrotum is loose skin which looks like sac it contain arteries and veins which supply blood to the reproductive system of the male 

This condition is very similar to varicose veins but there is little difference varicocele occur in scrotum ,it affect the vein known as pampiniform plexus and varicose veins occur in lower extremities


Risk factors 

  • There are various lifestyle activites which promote the unhealty testicle such as smoking ,alcohol ,taking excessive alcohol and coffee ,overweight or obese ,
  • Sometimes Varicocele may occur due to improper underwear

  • If it occurs at the age of or above 40 it may be occur due to blockage in larger veins of the abdominal area due to kidney tumor

Complications


  • The main complication of the varicocele is infertility , 35 to 44% of  primary fertility in men occur due to varicocele. 45 to 81 % with infertility varicocele is the cause of infertility .
  • Shrinkage of testicle – atrophy of the testicle occur due to malfunctioning of the valves 
  • Social stigma – sometimes it may lead to depression or other mental problems due to infertility or disturbance in male reproductive organ .

  • The exact cause of the varicocele is unknown but it believe it may be occur due to impaired in blood circulation in the scrotum.
  • Normally spermatic cord is present in male reproductive part ,spermatic cord contain arteries ,veins ,nerves , and tubes .
  • The main function of the spermatic cord to provide connection and circulate the blood to and from the testicles .
  • veins which is present in spermatic cord carry the blood flowing from the body back towards the heart , and spermatic cord also contains a bunch of valves in the veins , valves maintain the flow of blood in one direction and maintain from back flow of the blood .
  • In case of varicocele these valves become fail , as a result some flow of the blood goes back ,these backed up blood collected in pools in veins ,this collection of blood cause stretching of the veins , or swollen of the veins in the scrotum that’s why affected scrotum look bigger then the normal scrotum 

Diagnosis and treatment


  • Diagnosis of the varicocele is made by physical examination of the scrotum ,scrotum looks like a bag of worms ,which is nontender

What is cheilitis?


Cheilitis is a broad term which is used to describe the inflammatory condition of the lips,cheilitis also called chapped lips 

Cheilitis can occur in perioral skin(the skin around the mouth), vermilion border (the junction of the lips and skin), or labial mucosa (the mucous membrane lining the inner aspect of the lips.


Risk factors

  • Cheilitis may occur in response to allergy from anything such as lipstick, cosmetics, drugs, toothpaste, rubbers, metals such as nickel.
  • It may occur due to excessive exposure to sun, dry heat or cold weather.
  • Some internal reasons may trigger conditions such as autoimmune disorder, oral infection 
  • It is age-related, it is common in adults due to sagging of the skin
  • People with weak immune systems are more prone to developing cheilitis.
  • Generally, this condition is found in those people who spend more time outside such as landscapers, fishermen, or outdoors athletes, farmers, construction work, hikers sailors.
  • People who have light skin tone also prone to develop actinic cheilitis

Cause -it is mainly found in children and elderly people because it is occur due to excess drooling of saliva,due to collection of excessive saliva around the corners of the mouth it leads to a favourable environment for the development of the microorganism, such as yeast called candida, it is the most common cause 

Symptoms -this condition is characterised by cracks and fissures at the corners of the mouth, the formation of blisters, redness, and bleeding of the corners of the mouth, itching, scaly lips, sometimes crust may form around the corners of the mouth


Actinic cheilitis –this is an inflammation of the lips but usually it appears on the lower lip,it is caused by damage of the lip epithelium ,it is outer outermost layer of the lips.it a painless condition ,but it may lead to squamous cell carcinoma.

Cause –it is caused by long term exposure of sunlight, and also it is found in people who live in damp areas .

Symptoms-the initial symptoms of this type is dry and cracking of the lips ,as the condition develops there is formation of white scaly patch(leukokeratosis) on lip with red and swollen lip,it is generally occur on lower lips .tightness and wrinkles of the skin ,tenderness ,soreness,loss of demarcation between lower lip and surrounding skin


Granulomatous cheilitis-this condition is related to underlying disease such crohn’s disease, sarcoids ,  allergy , infection or cancer.

Symptoms-cracking of lips, headache, fever and maybe mild lymph node becomes enlarged 


Exfoliative cheilitis-it is a rare reactive condition and it manifests as continuous peeling of lips .

Cause-it occur due to poor oral hygiene ,bad habits such as constant picking or biting of the lips and sometimes HIV may associated 

Symptoms –initially there is formation of thickened surface layer ,there is peeling of the lips lips look red ,condition may become painful,causing difficulty in eating ,and speaking ,hemorrhagic crust may form after bleeding from lips dryness of lips ,tingling sensation and maybe sometime itching of the lips


Lichenoid cheilitis –this condition is found in person whos is already suffer from lichen planus ,lichen planus when affects the lips or skin around the lips .

Cause-the exact cause is unknown but it may be triggered after some medications such as beta blockers and nonsteroidal anti-inflammatory drugs ,and it is also genetic and weak immune system.

Symptoms-appearance ofwhite patches in mouth or web like threads  ,it may be slightly raised but it is painless condition.in some cases may ulcer develop in mouth which causes difficulty in eating ,drinking and become painful


Cutaneous lupus erythematosus-this type is associated with systemic lupus erythematosus , the main symptoms of SLE is butterfly rash if these rashes spread to mouth it cause scaling and formation of plaques on the lips.


Cheilitis glandularis-in this condition there is progressive enlargement of the lower lip ,this condition also associated with lining term exposure of sun rays ,smoking and repeated lips biting , due to this activity the delicate labial mucosa of the lower lip become swollen and tenderness.


Eczematous cheilitis –this condition is associated with some allergens such as lipstick or other cosmetic item. If person having allergy from this article it lead to itching ,cracking and peeling of the skin of the lips .this condition is also related with atopic dermatitis


DIAGNOSIS & TREATMENT


Diagnosis of the cheilitis is made  by

  • Detailed history taking -this step include proper history of patient regarding the problem such as onset of the disease,all symptoms,question about denture history,oral care,any medical history or any skin condition , any habit of smoking,bowel movement .
  • Physical examination-physical examination of the mouth is potential to make the diagnosis of the condition ,findings during physical examination are –

  • The exact cause of the fibroadenoma is not known but it is associated with the reproductive hormone ,possibly occur because of increased sensitivity to the hormone oestrogen .and the lump shrinks as the age increasing because at olde age the hormonal level decreases .
  • But there are some factors which triggered the condition such as pregnancy ,breastfeeding,before menses ,taking oral contraceptives ,at this time the size of fibroadenoma may increase .

DIAGNOSIS AND TREATMENT


Diagnosis


  • Diagnosis of fibroadenoma is made by physical examination of the breast in this examination both breasts is palpated by hand to find out the lump in the breast .
  • Family history of fibroadenoma also asked by the physician.
  • Breast ultrasound -in this technique sound waves is used to take a picture of the inside of the breast  
  • mammogram (x ray of the breast ) may be required to read the image of breast tissues .
  • Biopsy of the lump may be required to rule out the type and cancerous condition.
